Child Information Sheet – This form helps us keep our data base updated with all of the kids in our children’s ministry. Please make sure we have current contact information for your family.

Child Photo Release – We like to use photos of our Calvary Kids “in action” for newsletters, bulletin boards, and various slide shows for advertising our children’s programs. We need your permission to be able to use photos of your child. Please complete the attached photo release and turn it in at our children’s welcome area on Sunday mornings or drop it off to the church office.
